Home Based Care Project

Seeing that HIV/AIDS was becoming a major problem for a number of people, we felt so unstable about the amount of assistance we were offering to our HIV affected clients. Thus, a team of concerned and dedicated people conjoined to form what is known as a Home Based Care project to provide intensive support and care to people who are in any way affected by the virus and also requiring constant counseling and medication provision.

A team was identified to implement this programme. It consisted of the following members: -

Dr. Volker, Sr. Ndwandwe, Mr. Ntuli, Sr. Molefe and Mr. Nzimande. This team did rounds in both medical wards joined by a doctor and a sister. The rounds started at 08h30 and all HIV infected and affected patients were seen. There were forms that were designed for the programmme. i.e. daily checklist – a tool used to assess a patient on admission and on a daily basis, another form for post testing counseling or for patients refusing to be tested, and another form was for ongoing counseling for patients and relatives.

Responsibility of the Team

There were identified responsibilities that were stipulated and confirmed by all team members of the home-based care project. These are as follows:-

Continuous communication between members of the project and patients was then consolidated by a discharge ticket that was given to all discharged patients. This ticket consists of a telephone number for Drop in center where advice, counseling and support is offered. Discharged patients who are in need of our service are advised to use the number given on a discharge ticket to contact any of our members so as to acquire assistance.

This is an ongoing project that we think has given us enthusiasm to provide more support to HIV/AIDS infected and affected Clients.
