Vision:

To achieve optimum health for all people at risk, those living with chronic conditions, and older persons in KwaZulu-Natal

Mission:

To develop a co-ordinated, integrated, cost-effective, sustainable and customer orientated PCH package of services for preventing and managing chronic diseases, blindness and ageing.

Goals:

Develop a strategy to decrease the morbidity and mortality associated with chronic diseases and abuse. To improve the quality of life of older persons To eliminate all preventable causes of blindness

Objectives

To increase Human Resource capacity in the prevention, early detection and control of chronic diseases
To increase Human Resource capacity in managing conditions associated with ageing.
To restore the right to sight of all blind people
To change community habits detrimental to healthy lifestyle changes
To improve prevention, identification, management and reporting of elder abuse
