This is a comprehensive package of forms for paediatric and neonatal record keeping. There are forms specific to paediatrics, some specific to neonates, and some useable for both.

There are also instructions for implementation of this system, for using some of the forms, and suggestions for how to order the patient folders.

Use these forms as a way of improving quality of paediatric record keeping, and if you have suggestions for improvement, or a need for forms not in this package, please let us know.

Print out the forms and keep them as a master copy for your hospital stationers.

Do this by opening, then printing "Clinical Records: Complete Set".

Print out the catalogue and ordering forms and use these to order forms from your stationers. All forms (except 1 - see below) are A4 size, and some should be printed back to back. Make sure that when printing back to back, they are exactly the same as the originals.
