In order that we are able to provide you with prompt and efficient service you deserve when you visit our hospital for medical attention, we ask that you bring the following with you:

• Identity book (ID)
  
• Medical Aid Card
  
• Yours or next of kin’s contact telephone number
  
• Clinic card
  
• Medication you are taking presently
  
• Residential address
  
• Toiletries
  
• Money for transport back home or to phone someone to pick you up
  

In addition to this, and to ensure pleasant dealings with one another, when you visit our hospital you have a responsibility to:

• Treat health care workers and other patients with respect
  
• Comply with hospital policies and guidelines
  
• Provide correct information on identity and financial status
  
• Pay hospital fees as assessed
  
• Provide accurate history to health care workers
  
• Accept responsibility for decisions taken about your treatment
