Hlengisizwe Community Health Centre is situated at Hammersdale, Mpumalanga Township in eThekwini. It has a total of 31 beds. Hlengisizwe CHC serves the population that resides in the Hammersdale catchment areas.  
Hlengisizwe Community Health Centre was built by the KwaZulu-Natal Department of Health.  In April 1999 the then MEC for health Hon Dr Zweli Mkhize gave instructions that the facility be opened.

It directly serves a population of 132 503 and indirectly serves as a referral for 5 affiliation clinics, this amounts to a total population of 140 000.

The institution is committed to the Batho Pele principles and Patient Rights Charter.
