Pholela Community Health Centre is located in the Sisonke health district and was official opened on 21 April 2001.
Pholela CHC's vision is to be a leading provider of optimal community orientated primary health care in KwaHlanganani. This vision is supported by the CHC's mission which is to provide effective, efficient and integrated primary health care service for the community of KwaHlanganani through community orientated primary health care system.
The CHC serves the people of KwaHlangani and surrounding areas, up to Donnybrook. We have a catchment population 17 819. There are two mobile teams who visit about 20 points.
The CHC has achieved a certificate of recognition for the implementation of the comprehensive care plan for the prevention, treatment, care and support of HIV and Aids, STI and TB in KwaZulu-Natal. Pholela CHC abides by the Patients Rights Charter and Batho Pele Principles
